You may copy the text onto your own letterhead, or write your company name directly on the PDF.

A signed release is recommended for anyone photographed. It isn't needed when a similar release is already signed by an employee at the time of hire. A signed release matters because an employee could later claim they never granted permission to use their image - which could require pulling those images from use, or canceling an ad campaign entirely.
